TSA Survey 2026

We would really appreciate just a few minutes of your time to complete our 2026 survey. 

This is your chance to tell us what you want from us, what we’re getting right and what we can do better. You can help us focus on what matters most to you.

The survey is open to anyone aged 16 or over who lives with TSC and to family members and carers. More than one person from the same family can complete it.

And, if you provide your name and email address (and are aged over 16), you will automatically be entered into our draw to win one of three $100 eftpos gift cards. There will be three gift cards available and draws will be made from all survey entrants who complete the survey and provide their name and email address (excluding previous winners of a gift card).  The survey can only be completed once. Draws will be made on 26 October, 9 November and 30 November.  Winners will be notified via email.
